What is Self-Directed Living Supports?

Self-Directed Living provides support to help you develop, improve, and retain skills to live a successful life. We help with personal care/hygiene, light housekeeping, cooking, transportation, shopping, social skills, and more. We can assist you in your home, local community, at medical appointments, etc.

The level of support your receive depends on your specific needs, goals, and budgeted hours provided by the Medicaid waiver.

Interested in starting services?

Connect with us by visiting the Referrals Page. If you have a case manager, let them know you want to connect. The case manager will submit a referral through the portal.

Starting Services: Once the service(s) requested have been approved by the Executive Director of Self-Directed Living Supports a member of our team will contact you to discuss next steps for the intake process.

After the intake process: A DSP, Direct Support Professional, will be assigned to work with you depending on compatibility and availability.

ALL DSP requirements:
  • 18yrs old
  • Negative TB test upon hire
  • CPR/First Aid
  • Valid Driver’s license and auto insurance
  • Vehicle check
  • Annual training
  • Competency training on person specific plans
  • Ongoing training

Funding Options for this service:

This service is paid for through the Indiana home and community-based services Medicaid Waiver. Private pay options may be available upon request with a contractual agreement.
