
Your Right to Choose
Any person that receives services may terminate services at any point and can do so by contacting their Case Manager. People are freely given the choice of providers and services and may decide to terminate with OPG, Inc. to seek another provider or to discontinue that service.
Termination by OPG, Inc.
OPG, Inc. may also decide to terminate services with a person. Situations that could result in termination of services by OPG, Inc. may include, but are not limited to:
- Insufficient budget per the NOA to provide for required level of care
- Issues that could result in high risk for OPG, Inc. and that may be outside of our scope of practice or inconsistent with OPG’s Mission Statement
- Issues that were not disclosed during intake process that could pose a risk to a member of staff or to OPG, Inc.
- A person’s lack of participation with or inability to cooperate with identified goals and model of support
- Inability to provide services due to geographical location
- Conflict of interest
If OPG, Inc. decides to terminate services, the following process will occur:
- The CEO or designee will send official termination letters to the person, the guardian (if applicable), and the Waiver Case Manager. The Waiver Case Manager is required to provide the family a pick list. OPG, Inc. will only continue providing support for sixty (60) days after notice has been given. If no suitable provider is selected in that time, OPG, Inc. will continue providing services until a provider has been found and a transition of services has occurred.
- When services are terminated, all members of the IST will be informed. If a provider has not been selected, OPG, Inc. is required to continue to support the person until a provider is found, per Indiana Administrative Code.
